General Statement of Duties Leads marketing efforts as well as assists with the administration of the admissions process for a skilled nursing facility providing short term rehabilitation and long-term care services.
Reports to the Director of Admissions.
Qualifications Education and Experience: Bachelor’s degree in Nursing or Social Work from an accredited college or university, plus three (3) years of related admissions and marketing experience on behalf of a health care facility, or equivalent combination of education and experience.
Qualifications: Demonstrated knowledge of the key aspects and regulatory requirements of long-term care.
Proven ability to organize, schedule and prioritize work.
Proven ability to work independently and exercise sound judgment.
Demonstrated ability to deal tactfully and courteously with residents, and families.
Proven ability in the use of a personal computer, word processing and related Microsoft Office applications.
Examination: 100% Training & Experience Assessment The examination to determine a candidate's eligibility for the position will be a Training and Experience Assessment.
This type of examination requires that you completely fill out the application, adding pages for additional relevant information where needed, and attaching any documentation of certificates, degrees, other significant documents, etc.
A candidate's eligibility to be interviewed will be based solely on material submitted at the time of application.

Job Description Department: Nathaniel Witherell Bargaining Status: LIUNA Salary Range: F FSLA Status: Exempt Date Amended: Jan 2024 Duties and Responsibilities: Serves as a marketing liaison with hospitals, other nursing facilities, assisted living facilities and all relevant referral sources.
Works closely with case managers, physicians and families to ensure appropriate placement for patients/residents.
Maintains communication with hospital discharge staff, administrators, other nursing homes, physicians, rehabilitation centers and home care agencies.
Coordinates with medical, nursing and accounting staff to ensure smooth transition for patient/resident admission.
Meets with prospective residents and/or their families regarding admissions, conducts tours of the facility and responds to requests for information on services.
Documents all applications and inquiries, ensuring regulatory compliance with documentation requirements relative to applications.
Maintains and updates Waiting List and Inquiry Files providing ongoing communication with prospective residents.
Reviews and distributes new applications to appropriate divisions and agencies for pre-admission evaluation to ensure appropriate placement for patients/residents.
Coordinates the admission process with families and/or patients/ residents, ensuring the appropriate documentation has been submitted and required information provided.
Coordinates with medical, nursing and accounting staff to ensure smooth transition for patient/resident admission.
Prepares initial admissions data entry, medical chart and admissions documents and distributes executed documents to the appropriate disciplines.
Follows up with patients/ residents, and families upon patient admission and discharge.
Communicates with the hospital when residents are transferred for care in order to evaluate resident needs on return.
Interacts with the Director of Admissions and, as required, nursing administrators/supervisors relative to medical/financial issues that impact upon admission decisions.
Conducts insurance verification including pre-certification and pre-authorization with payment sources.
Remains current with all insurance requirements.
Treats patient/resident information according to the appropriate HIPAA Privacy Regulations.
Provides administrative support to the Director of Admissions.
Monitors inventories of supplies and marketing materials to facilitate timely orders.
Performs related work assigned.
Supports Town policies and philosophies.
Supplemental Information All positions may be subject to an interview process.
The Town reserves the right to limit the number of qualified candidates who will be invited to participate in the examination, or any part thereof.
All outside candidates will be required to undertake and successfully pass a pre-employment medical examination, which includes a substance abuse test that includes testing for THC (the psychoactive ingredient in marijuana), given at the Town's expense prior to employment if conditionally offered a part time, full time or safety sensitive position.
Candidates for employment at Nathaniel Witherell Skilled Nursing Facility are required to demonstrate their vaccination status against COVID 19.
The Town of Greenwich has implemented a policy of background investigations for all full and part time positions positions prior to hiring as part of the reference checking procedures for outside candidates.
The investigation will only be conducted as the last step prior to an offer.
Refusal to sign the release form will terminate the candidate's further consideration.
